Comments: This systematic review suggest that the malignancy rate as detected with colonoscopy after imaging-proven uncomplicated acute diverticulitis is low.

Comments: The natural history of uncomplicated diverticulitis is often benign. Of 3165 patients hospitalized for acute diverticulitis, only 13.3% had one and 3.9% a second recurrence within 8.9 yrs. 20-35% progress to chronic pain compared with 5%-25% when treated operatively. Antibiotics and fiber were not as beneficial as previously thought, mesalamine might be useful.
