Barrett esophagus and risk of esophageal cancer: a clinical review.

Comments: The annual risk of esophageal cancer is approximately 0.25% for patients without dysplasia and 6% for patients with high-grade dysplasia. Endoscopic screening: for Barrett esophagus without dysplasia at intervals of 3 to 5 yrs. Endoscopic eradication therapy is the treatment of choice for high-grade dysplasia and is an option for low-grade dysplasia. Endoscopic eradication therapy is not recommended for the general population with nondysplastic Barrett esophagus.

 

Outcomes of treatment for achalasia depend on manometric subtype.

Cigarette smoking increases risk of Barrett's esophagus: an analysis of theBarrett's and Esophageal Adenocarcinoma Consortium.

Comments: Cigarette smoking is implicated in the etiology of esophageal adenocarcinoma, and has now been shown to predispose to Barrett's esophagus with a RR of 1,6 in 5 case-control studies comparing 1059 subjects with Barrett's esophagus with 1332 from the reflux disease controls, and with 1143 population-based controls.Increasing pack-years of smoking increased the risk for Barrett's esophagus, and there was a synergy between ever-smoking and heartburn or regurgitation

 

Targeting unlesioned pharyngeal motor cortex improves swallowing in healthyindividuals and after dysphagic stroke.

Comments: Patients with stroke experience swallowing problems (dysphagia); increased risk of aspiration pneumonia, malnutrition, and dehydration; and have increased mortality. A new neurostimulation technique (paired associative stimulation [PAS]), combining peripheral pharyngeal (electrical) with cortical stimulation (of the unlesioned brain) demonstrated beneficial neurophysiological and behavioral effects on swallowing function determined by videofluoroscopy in healthy individuals and patients with dysphagia from stroke. Further investigation into the use of neurostimulation is warranted.

Comments: The discovery of the "acid pocket", an area of unbuffered gastric acid that accumulates in the proximal stomach after meals and serves as the reservoir for acid reflux in healthy individuals and gastroesophageal reflux disease (GERD) patients. However, there are differentiating features between these populations in the size and position.

Comments: When the normal passage of food along the digestive tract is hampered, feeding via a nasogastric tube (NGT) or via percutaneous endoscopy gastrostomy (PEG) is possible. The evaluation of 9 randomised controlled studies demonstrated a slightly lower intervention failure in the PEG (12 %) than in the NGT (23%) group (P = 0.01). There was no significant difference in complications.

Comments: In this trial of children with poorly controlled asthma without symptoms of GER who were using inhaled corticosteroids, the addition of lansoprazole, compared with placebo, improved neither symptoms nor lung function but was associated with increased adverse events.

Comments: Symptom relief in reflux disease (GERD) has been obtained with radiofrequency delivery (Stretta procedure) at the gastro-esophageal junction (GEJ), but the mechanism remains unclear. Active and sham Stretta was compared in 11 pts each. Three months after the initial Stretta, no changes were observed in esophageal acid exposure and lower esophageal sphincter (LES) pressure, but the symptom score was significantly improved and GEJ compliance (which reflects altered LES neuromuscular function) was decreased. Sildenafil, an esophageal smooth muscle relaxant, normalized GEJ compliance again to pre-Stretta level, arguing against GEJ fibrosis.GEJ compliance.may thus have contributed to symptomatic benefit by decreasing refluxate volume.

 

Clinical, but not oesophageal pH-impedance, profiles predict response to protonpump inhibitors in gastro-oesophageal reflux disease.

Comments: No reflux pattern demonstrated by 24 h pH-impedance monitoring is associated with response to PPIs in GERD. Of 100 patients with typical GERD 43 responders and 57 non-responders to PPI were studied. Overall, multivariate analysis showed that absence of response was absence of oesophagitis (p=0.050), body mass index (BMI) =25 kg/m(2) (p=0.002), functional dyspepsia (p=0.001). and irritable bowel syndrome (p=0.045).
